MENTAL STATE EXAMINATION

Appearance: Janet is an 18-year-old Caucasian woman. She is about 160cm tall and weighs 54 kg. Her hair is tangled and greasy. She has dressed in a dirty singlet, mini skirt and thongs notwithstanding the fact that it is winter. She appears pale and has lost weight as a result of going for several days without eating. She has a poor level of personal care and grooming. She has gone for several days without showering and changing her clothes. This is out of character for Janet, who was previously appropriately dressed and well groomed.

Behavior: Janet is irritable and angry. She is silent when one tries to intervene with her although she is compliant with interventions. She has her head lowered and will only make intermittent eye contact.

Speech: Janet’s speech was of a loud volume and pitch as well as the rate as she yowls and scowls when being spoken to. She screams to her parents when they talk to her.

Mood: She seems depressed as she lost her very close cousin. She takes alcohol to deal with her stress and locks herself up in her room and refuses to talk to anyone. She was not a heavy drinker in the past and never used to take drugs.

Affect: Janet does not feel relaxed around people anymore and will more often than not start shaking when she meets new people. She is quite nervy and distant, and this has led to the end of her relationship with her boyfriend.

Form of Thought: Janet does not talk much, and she does not clearly express herself concerning her problems. She feels she is rotten. She believes medical help is not enough and cannot be possibly be helped by any medical therapies, and she does not want to be at the hospital.
